Onward Transitions is part of an incredible network of providers in the Portland area. We are not a specialized recovery program, nor are we designed for those requiring acute care. We practice generalized living skills and provide meals, a safe community, therapy and support, career and vocational coaching, apartment monitoring and visitation, social activities, community outreach, and 24-hour on-call. Through our evidence-based relationship model we connect young adults to an actual "hand-shake" network, and to a face-to-face community. We live independently, share a neighborhood philosophy, connect with a vibrant social community, practice communicating effectively, demonstrate a solid work ethic, and build developmental relationships over time: over our dinner table nightly, and with family and friends at school, work, and at play in the city of Portland, Maine. Co-founders Margie Lannon (Business Development), Darrell Fraize (Clinical Director) and Tracy Bailey (Executive Director) bring a combined 70 years of experience to the emerging adults enrolling in the service.
