Living Well Transitions is a coed therapeutic program for ages 18 and older, designed to help clients reduce unproductive or high-risk behaviors. The program opened in June 2004 and currently accepts approximately 15 clients. Under the supervision of clinical staff, the clients live independently in apartments within the Boulder community in what the staff describes as a "real-world living environment."

Living Well Transitions believes that "if clients are not distracted by the group dynamic, they are better able to focus on their personal goals while increasing their opportunity for experiential life skills and gaining a sense of personal responsibility."

The clinical team consists of licensed professional counselors, pre-graduate interns in the field of Psychology and psychotherapists actively working towards licensure. They interpret psychological evaluations, consult with psychiatrists and medical treatment programs, and can work with a variety of psychological and behavioral disruptions and diagnoses.

The Living Well program is based on 'Five Elements of Transition.' These 'Elements' teach clients how to rent an apartment and buy furniture, manage their household, attend to medical issues, personal hygiene, diet and general fitness and learn how to handle their finances. The clients also receive assistance in attaining educational goals, finding and keeping a job, developing a resume, the application process, interview techniques and ongoing support after the client has secured a job. Additional elements include encouraging clients to build relationships and to pursue hobbies through a community network that includes artists, musicians, and athletes.

Transition Plans may include individual counseling or therapy, family counseling, educational and vocational assistance, tutoring and placement support, life-skills training, social skills enhancement and community networking.
