Trilby Hoover announces that she is leaving the Ascent Wilderness to open an aftercare support service designed for adolescents and young adults. The program called Sheepgate Services (aka SGS), is designed to create a system of accountability for young people in transition following completion of substance abuse treatment.
Trilby Hoover has more than 27 yrs of professional experience working with families. She is a psychiatric nurse with an extensive background working with adults and minors in every area of behavioral health, including inpatient and outpatient chemical dependency treatment. In addition, she is experienced with the addictive process and has been in active twelve step recovery since 1983. Trilby has discovered in life that growth comes with new challenges. She is always ready to utilize her professional and personal recovery background to provide assistance to families. Trilby is joined by Jill Nugent and a team of experienced recovery support partners. "We see this as an opportunity to partner with the professionals providing seamless transition services for young clients progressing into healthy active lifestyles".
Sheepgate Services (SGS) implements the components of aftercare planning into a practical format for accountability. Services are all-inclusive, providing nation-wide access (scheduling and documentation of) weekly UADS. We record contact between the client, SGS and case monitored activities as outlined in the aftercare plan. Some examples of the activities identified in the aftercare plan include 12 Step meeting attendance, therapy appointments, Psychiatrist visits, aftercare support group attendance etc. SGS provides documentation of monitoring activities to client and identified stakeholder(s) monthly and will initiate contact with stakeholders when there is a negative development in client's monitored activity. Our goal is to support intervention before there is significant relapse.
